I went from Phnom Penh to Vietnam by river. You can do it in reverse -- Delta Adventure Tours in HCMC (or any other travel agent or hotel, probably) sells either a three-day tour of the Mekong ending up in Phnom Penh, or a quicker two-day version. To take the boat without a tour (which is what we did, but coming from Cambodia), you would need to hop a bus to Chau Doc, then boat from there.

I did the 3-day landcrossing in August with Delta Adventure Tours http://www.deltaadventuretour.com/. The first day is sightseeing in the Mekong Delta. The 2nd day was a bit boring, with 5 hours on a noisy boat - make sure you take a book. The company will hit you for extra costs along the way - eg breakfast at the hotel where they get commission. Dinner the 2nd night at their hotel, where there is no other shops for miles etc. Overall great way to get to Cambodia.

If travelling from Phnom Penh up to Siem Reap, take the bus and book 2 seats ($8) and enjoy the 6-7 hour journey, heaps cheaper than the boat, and more comfortable!n
